Job summary

Somatus is hiring an RN Care Manager to join our Chronic Kidney Disease and End-Stage Kidney Disease program. You will coordinate in-home visits, dialysis clinics, and physician offices to support high-need patients and reduce readmissions.

You will educate patients, collaborate with clinicians, and develop individualized care plans, traveling regionally up to 75% of the time. Requires RN license, 2+ years of RN experience, BLS, and CKD/dialysis exposure.

Qualifications

• RN license required and ability to get licensed in other states as needed.
• 2+ years of RN experience in a multidisciplinary team with physicians.
• Valid BLS certification from an accepted training facility.
• Renal, CKD or dialysis care experience as a main focus of your job.
• Residence in a location with high-speed internet or access to reliable service.
• Willingness to travel to partner hospitals, clinics and facilities within your region.

Responsibilities

• Outreach and scheduling face-to-face visits with members in homes, dialysis clinics, or physician offices.
• Support care planning and deliver education to patients to improve outcomes.
• Collaborate with nephrology practices to enhance collaboration and patient care.
• Develop and implement individualized care plans with multidisciplinary teams.
• Travel regionally to meet with providers and patients, up to 75% of the time.
