Job summary

Somatus is seeking an RN Care Manager to lead VBC efforts focused on high-need CKD and ESKD patients. You will collaborate with nephrology teams, plan and coordinate in-home visits, and educate patients to improve outcomes.

This field-based role requires travel within your region, strong care coordination, and excellent communication with providers and patients. You’ll join a team committed to evidence-based, patient-centered care and must have RN licensure, BLS, and experience in

Qualifications

• RN license and ability to get licensed in other states as needed.
• 2+ years of RN experience in a multi-disciplinary team setting.
• Valid BLS certification from a licensed facility.
• Renal, CKD or dialysis care experience as a main focus.
• Willingness to travel within assigned region to support care coordination.

Responsibilities

• Perform home, facility, and clinic visits to support high-risk patients.
• Develop individualized care plans with nephrology and interdisciplinary team.
• Educate patients and families on discharge planning and self-management.
• Coordinate with provider practices to optimize workflows and communications.
• Travel up to 75% to meet patients in homes and clinics.
