Job summary

Somatus is seeking an RN Care Manager to lead CKD/ESKD focused care coordination with physicians and patient health advocates. You will conduct home visits and coordinate with nephrology teams to improve patient outcomes.

Travel 75% of the time; RN license required and strong collaboration skills are essential. We offer PTO, professional development, and a supportive, patient-centered culture.

Qualifications

• RN license; ability to obtain multi-state licensure as needed.
• 2+ years RN experience in multi-disciplinary settings with physicians.
• BLS certification from a licensed facility.
• Renal CKD or dialysis care experience as main focus.
• Must reside where high‑speed internet is available or have access to it.
• Willingness to travel to partner hospitals, clinics and community facilities.

Responsibilities

• Partner with physicians and practice staff to improve patient outcomes.
• Coordinate care planning and regular interdisciplinary care team meetings.
• Educate patients and provider offices; develop strong care plans.
• Manage transitions of care and reduce readmissions.
• Direct and mentor non-licensed and licensed team members as needed.
