Job summary

Somatus is seeking a Respiratory RN Care Manager, VBC-like role focused on high-need CKD/ESKD patients. You will partner with nephrology practices, schedule visits, and perform comprehensive assessments to support coordinated, patient-centered care.

Travel will be required to member homes, dialysis clinics, and physician offices within the assigned region, with strong emphasis on interdisciplinary collaboration and education of care plans.

Qualifications

• RN license and ability to get licensed in other states as needed.
• 2+ years of RN experience, including working as part of a multi-disciplinary team and Physicians.
• Valid BLS certification from an AHA or American Red Cross facility.
• Renal, CKD or Dialysis care experience as main focus of your job.
• Willingness to travel within your assigned region to support care coordination.

Responsibilities

• Partner with physicians and practices to establish collaborative relationships focused on improving patient outcomes.
• Support care team in planning and coordinating regular interdisciplinary care team meetings.
• Build positive relationships with patients and provider offices through in-person and telephonic engagement.
• Develop partnerships with provider teams to support clinical and operational goals.
• Collaborate with practices to optimize workflows aligning with care team processes.
• Educate provider practices on the Somatus program and integrated workflows.
• Provide complete continuum of quality care via in-person, telehealth or phone interactions.
• Travel to member homes, facilities, and offices for visits and care planning.
