Job summary

Somatus is seeking an RN Care Manager to focus on high-need CKD/ESKD patients, coordinating care with nephrologists, PCPs, and interdisciplinary teams to improve outcomes. You will conduct outreach, schedule home visits, provide care planning education, and travel to member homes, dialysis clinics, and offices to support high-risk patients and prevent readmissions.

Required: RN license, 2+ years RN experience, BLS, renal care experience; BSN or higher preferred; strong communication, HIPAA

Qualifications

• RN license required and eligible for multi-state licensing.
• Minimum 2 years RN experience in a multidisciplinary setting.
• BLS certification from a licensed facility.
• Renal CKD or dialysis care experience preferred.
• Willingness to travel across the assigned region.
• Ability to use MS Office and telecommunication devices.

Responsibilities

• Partner with physicians and practices to improve patient outcomes.
• Schedule home visits and care plan education.
• Travel to member homes, clinics, and offices up to 75% of the time.
• Educate patients on discharge requirements and self-management.
• Provide guidance to non-licensed and licensed care team members.
• Develop individualized care plans with nephrologists and PCP.
• Support transitions of care and reduce readmissions.
• Maintain HIPAA privacy and data entry proficiency.
• Perform other duties as assigned.
