Job summary

Somatus is seeking a RN Care Manager to support high-needs CKD and ESKD patients through comprehensive in-home, clinic, and telehealth care coordination. You will partner with nephrology practices to deliver integrated, outcomes-driven care and empower patients with education and self-management strategies.

This field-based role requires travel within the assigned region and collaboration with interdisciplinary teams to optimize workflows and improve patient outcomes.

Qualifications

• RN license in good standing; ability to obtain licenses in other states as needed.
• 2+ years RN experience in multidisciplinary teams.
• BLS certification from a licensed facility.
• Renal/KD or dialysis care experience preferred.
• Must reside in an area with reliable high-speed internet; able to travel within assigned region.

Responsibilities

• Partner with physicians and practice staff to improve patient outcomes.
• Lead care coordination meetings with partner practices.
• Conduct home, clinic, and facility visits to support high-risk patients.
• Educate patients and families on care plans and disease management.
• Develop individualized care plans with nephrologists and PCMs.
• Coordinate transitions of care and reduce readmissions.
• Provide guidance to non-licensed and licensed team members.
• Travel as required to member homes and clinics.
