Job summary

Somatus is looking for an RN Care Manager to support high-need CKD/ESKD patients through home visits and clinic partnerships. You will collaborate with nephrologists and care teams to coordinate comprehensive, patient-centered care.

The role requires RN licensure, BLS certification, and 2+ years of multidisciplinary team experience with dialysis care exposure. Travel within the region is expected.

Qualifications

• RN license and ability to get licensed in other states as needed.
• 2+ years of RN experience, including working as part of a multi-disciplinary team and Physicians.
• Valid BLS certification from a licensed facility.
• Renal, Chronic Kidney Disease or Dialysis Care experience as a main focus of your job.
• Reside in a location that can receive a high-speed internet connection or can leverage existing high-speed internet service.
• Comfortable traveling to partner hospitals, clinics, and community-based facilities within your assigned region; regular local travel is a key part of this role.

Responsibilities

• Outreach to patients and families to coordinate care in home, clinic, and facility settings.
• Schedule and conduct face-to-face visits with members to support high-risk patients.
• Collaborate with nephrology practices to improve patient outcomes and care coordination.
• Educate patients on discharge planning, medications, and disease management.
• Lead or participate in interdisciplinary team meetings to align on care plans.
