Somatus is seeking an RN Care Manager, VBC, to support high-need CKD and ESKD patients across facilities and in home settings. You will partner with nephrology practices, physicians, and the care team to deliver coordinated, person-centered care.

The role emphasizes field visits, education, and collaboration to improve outcomes, with regular travel within the assigned region and a strong focus on care coordination and patient engagement.

Qualifications

• RN license and ability to license in other states as needed.
• 2+ years RN experience in multi-disciplinary teams.
• BLS certification from AHA or Red Cross.
• Renal CKD or dialysis care experience as a main focus.

Responsabilités

• Outreach and care coordination for high-need CKD/ESKD patients.
• Schedule visits at homes, dialysis clinics, and physician offices.
• Travel within assigned region to support care plans and education.
• Educate patients on discharge and renal condition management.
• Provide clinical guidance to non-licensed and licensed team members.
