Job summary

Somatus is seeking an RN Care Manager to support high-need CKD and ESKD patients across the region. You will lead outreach, schedule visits, and coordinate care in person, at clinics, or via telehealth in collaboration with the care team.

The role travels within the region and partners with nephrologists and PCPs to improve outcomes. A valid RN license and BLS are required; BSN or higher and multi-state licensing are a plus.

Qualifications

• RN license and ability to get licensed in other states as needed.
• 2+ years of RN experience, including working as part of a multi-disciplinary team and Physicians.
• Valid BLS certification from a licensed AHA or American Red Cross facility.
• Renal, Chronic Kidney Disease or Dialysis Care experience as a main focus of your job.
• Reside in a location with high-speed internet or access to it.
• Willing to travel to partner hospitals, clinics, and community facilities within your region; regular local travel is essential.

Responsibilities

• Outreach, scheduling face-to-face visits with members in homes, dialysis clinics, or physician offices.
• Travel to member homes, facilities, and clinics to support care coordination.
• Develop and implement individualized care plans with nephrologist, PCP, and interdisciplinary team.
• Collaborate with physicians and practice staff to establish collaborative relationships and improve outcomes.
• Provide care coordination, education, and education to patients and families.
• Assist with transitions of care to reduce readmissions and improve health outcomes.
