Job summary

Somatus is seeking an RN Care Manager to support CKD and ESKD populations across assigned regions. You will partner with nephrology practices and primary care teams to deliver comprehensive, outcomes-focused care.

This field-based role requires in-home visits, clinic engagements, and strong collaboration with the interdisciplinary team. The ideal candidate holds an active RN license, 2+ years of RN experience in care coordination, and a current BLS certification.

Qualifications

• Licensed RN with active credentials in the states served.
• Minimum 2 years of RN experience, preferably in care coordination.
• Current BLS certification from a recognized provider.
• Experience traveling to hospitals, clinics and home settings.
• Familiar with HIPAA and patient privacy requirements.
• Strong teamwork across multidisciplinary teams.

Responsibilities

• Coordinate care with nephrology teams and primary providers.
• Schedule in-home visits and care planning for high-risk patients.
• Educate patients on discharge plans and CKD management.
• Develop and implement individualized care plans with clinicians.
• Travel within assigned region to support care coordination.
• Lead or participate in interdisciplinary care meetings.
• Communicate clearly with patients, families, and providers.
