Job summary

Somatus is seeking an RN Care Manager, VBC to focus on high-need CKD and ESKD patients. You will collaborate with nephrology practices, coordinate visits at home, clinics, and offices, and help patients navigate their treatment plans.

This role requires an active RN license, 2+ years of multi-disciplinary teamwork, BLS, and willingness to travel within your region. A BSN and CCM are preferred; strong communication and data-entry skills are essential.

Qualifications

• RN license to practice in the state; ability to obtain licenses in other states as needed.
• 2+ years RN experience within a multi-disciplinary team.
• BLS certification from a licensed facility.
• Renal CKD or dialysis care experience.
• Residence in an area with reliable high-speed internet access.
• Willingness to travel locally to partner hospitals, clinics and facilities.

Responsibilities

• Outreach and coordinate care with patients and partner practices.
• Schedule home visits, dialysis clinics, and physician offices.
• Travel within the assigned region to support high-risk members.
• Develop individualized care plans with nephrology and primary care teams.
• Lead interdisciplinary care team meetings to improve patient outcomes.
