Job summary

Somatus is seeking an RN Care Manager focused on high-needs CKD and ESKD patients. You will visit members in homes, dialysis clinics, and physician offices to support care planning and education, working with nephrology teams to improve outcomes.

You will coordinate with multidisciplinary staff, travel regularly, and empower patients through coordinated, person-centered care plans and self-management support.

Qualifications

• RN license and ability to obtain licensure in additional states as needed.
• 2+ years RN experience, preferably in multidisciplinary care teams.
• BLS certification from an approved training facility.
• Renal, CKD or dialysis care experience preferred as core to the role.

Responsibilities

• Collaborate with physicians and the care team to coordinate patient care.
• Travel to member homes, clinics, and hospitals to conduct visits.
• Assess barriers to care and develop individualized care plans with the team.
• Provide education and self-management support to patients and families.
• Support transitions of care and reduce preventable readmissions.
• Document interactions and outcomes for continuous care improvement.
