Job summary

Somatus is seeking a dedicated RN Care Manager, VBC to support high-needs CKD and ESKD patients, performing home visits and coordinating with nephrology and primary care teams to ensure adherence to care plans.

You will collaborate with physicians, health plans, and clinics, travel to member homes and facilities, and lead education to improve health outcomes and patient engagement within a comprehensive care program.

Qualifications

• RN license required and ability to obtain licensure in other states as needed.
• BSN or higher degree preferred.
• Experience working in multidisciplinary teams is essential.
• Strong communication and clinical assessment skills are required.

Responsibilities

• Conduct home visits, dialysis clinics, and physician offices to support high-risk patients.
• Develop individualized care plans with nephrologists, PCPs, and care teams.
• Educate patients on discharge care requirements and self-management.
• Coordinate with providers to improve care transitions and reduce readmissions.
