Job summary

Somatus is seeking an RN Care Manager to support high-need CKD and ESKD patients across our network. You will work with nephrology practices, coordinate in-person and telehealth visits, and develop individualized care plans to improve outcomes.

In this field-based role, you will travel to member homes, clinics, and partner hospitals, collaborating with interdisciplinary teams and educating patients on self-management. A valid RN license and BLS are required.

Qualifications

• RN license and eligibility to obtain licensure in required states.
• Minimum 2 years of RN experience in multi-disciplinary teams and patient care.
• BLS certification from an approved facility.
• Experience in renal, CKD, or dialysis care is preferred.

Responsibilities

• Outreach to high-need CKD/ESKD patients to coordinate care.
• Schedule face-to-face visits at home, dialysis clinics, or physician offices.
• Partner with nephrology and PCP teams to implement care plans.
• Educate patients and families on discharge and self-management.
• Support care coordination by participating in interdisciplinary meetings.
• Travel within assigned region to support providers and patients.
