Job summary

Somatus is seeking an RN Care Manager, VBC, to coordinate care for high-need CKD and ESKD patients across the care continuum. You will partner with nephrology practices, health plans, and providers to deliver home and clinic-based care, ensuring adherence to treatment plans and improving health outcomes.

You will travel within your region to member homes, dialysis clinics, and partner offices, providing education, care coordination, and interdisciplinary support.

Qualifications

• RN license in good standing; ability to obtain license in needed states.
• Minimum 2+ years RN experience in a multidisciplinary setting.
• Valid BLS certification from an approved provider.
• Renal, CKD, or dialysis care experience preferred.
• Reliable high-speed internet access and ability to travel within assigned region.

Responsibilities

• Conduct home visits, facility visits, and clinic assessments to support care coordination.
• Develop and implement individualized care plans in collaboration with nephrology practices.
• Educate patients and families on discharge planning, self-management, and care transitions.
• Coordinate with physicians, nurses, and allied staff to improve patient outcomes.
• Travel up to 75% of the time to partner sites and member homes.
