Job summary

Somatus is seeking an RN Care Manager for high-needs CKD/ESKD populations. You will travel to patient homes, clinics, and offices to coordinate care with physicians and the interdisciplinary team.

Strong communication and patient education are essential as you develop personalized care plans and support transitions of care. Required: RN license, BLS, 2+ years RN experience, and willingness to travel; BSN or higher and CCM preferred.

Qualifications

• RN license required and eligible for activations in needed states.
• BLS certification from an approved provider is required.
• 2+ years RN experience in multidisciplinary settings preferred.
• Experience in renal/CKD or dialysis care is a plus.
• Willingness to travel within the assigned region regularly.

Responsibilities

• Outreach and care coordination for high-priority patients in CKD/ESKD programs.
• Schedule home visits, dialysis clinics, and physician offices as needed.
• Develop and implement individualized care plans with the team.
• Educate patients and families on discharge care and self-management.
• Collaborate with nephrologists, PCPs, and care teams to improve outcomes.
• Support handoffs and transitions of care to minimize readmissions.
