Job summary

Somatus is seeking an RN Care Manager to coordinate care for high-needs CKD/ESKD patients. You will partner with nephrology practices and the care team to plan visits, educate patients, and implement personalized care plans.

You will travel to patient homes and clinics to support care coordination, barrier assessments, and patient education, while collaborating with physicians and the broader care team.

Qualifications

• RN license and ability to get licensed in other states as needed.
• 2+ years of RN experience, including working as part of a multi-disciplinary team and Physicians.
• Valid BLS certification from a licensed facility.
• Renal, Chronic Kidney Disease or Dialysis Care experience as a main focus of your job.
• Reside in a location with high-speed internet or can leverage existing service.

Responsibilities

• Schedule face-to-face visits with members in homes, dialysis clinics, and physician offices.
• Collaborate with nephrology practices to improve patient outcomes.
• Travel to member homes, facilities, and clinics to conduct visits.
• Develop and implement individualized care plans with the interdisciplinary team.
• Educate patients on discharge requirements and renal condition.
• Provide clinical guidance to non-licensed and licensed team members.
