Job summary

Somatus is seeking an RN Care Manager to support high-need CKD and ESKD populations through home visits, care coordination, and education. You will collaborate with nephrology practices and care teams to develop individualized care plans and improve patient outcomes.

The role requires RN licensure and the ability to travel up to 75% of the time; strong communication skills and experience with multidisciplinary teams are essential.

Qualifications

• RN license and ability to get licensed in other states as needed.
• Renal, CKD or dialysis care experience a main focus of the job.
• Willingness to travel across the assigned region 75% of the time.
• Strong communication and collaboration with physicians and care teams.

Responsibilities

• Perform home visits, care planning, and care coordination with nephrology practices and providers.
• Educate patients and families on discharge instructions and self-management.
• Lead and participate in interdisciplinary care team meetings and care coordination activities.
• Develop individualized care plans to address barriers to care and improve outcomes.
• Travel to member homes, clinics, and offices to support high-risk patients.
