Job summary

Somatus is seeking an RN Care Manager to support Chronic Kidney Disease and End-Stage Kidney Disease patients. This role entails direct patient outreach, education, and collaboration with healthcare providers to improve outcomes.

Qualifications

• RN license and ability to get licensed in other states as needed.
• 2+ years of RN experience, including working with multi-disciplinary teams.
• Renal, Chronic Kidney Disease, or Dialysis Care experience preferred.
• Must be comfortable with regular travel to partner hospitals and clinics.
• Valid BLS certification ONLY from a licensed AHA or American Red Cross.

Responsibilities

• Work closely with patients and physician practices to provide care.
• Conduct home visits and care assessments for high-risk members.
• Establish collaborative relationships with providers for better outcomes.
• Educate practices on the Somatus program and workflows.
