Job summary

Somatus is seeking an RN Care Manager focused on CKD/ESKD to coordinate high-need patient care across home, clinic, and facility settings. You will work with nephrology practices and the care team to implement individualized plans and improve outcomes.

The role requires RN licensure with multi-state mobility, BLS certification, and at least 2 years of multidisciplinary experience. Travel is 75% within the assigned region to support care coordination.

Qualifications

• RN license; ability to get licensed in other states as needed.
• 2+ years of RN experience, including working with a multi-disciplinary team and physicians.
• Valid BLS certification from an approved facility.
• Renal, CKD or Dialysis care experience as main focus.
• Location with reliable high-speed internet or access to service.
• Willingness to travel to partner hospitals, clinics and facilities; regular local travel required.

Responsibilities

• Perform home, facility, and clinic visits to support higher-risk members.
• Collaborate with nephrology, PCP, and interdisciplinary care team to implement care plans.
• Educate patients on discharge requirements, renal condition and self-management.
• Develop and implement individualized care plans with the team; remove barriers to care.
• Coordinate with provider practices to optimize workflows and care coordination.
• Provide guidance to licensed and non-licensed team members; delegation as appropriate.
