Job summary

Somatus is seeking an RN Care Manager to coordinate high-needs CKD/ESKD patients across home, clinic, and facility settings. You will work with nephrologists and primary care teams to implement individualized care plans and ensure smooth care transitions.

This field-based role requires regional travel, strong clinical judgment, and active patient education while supporting program goals and using MS Office tools to document outcomes.

Qualifications

• RN license and ability to get licensed in other states as needed.
• 2+ years of RN experience, including multidisciplinary teams and physicians.
• Valid BLS certification from a licensed provider.
• Renal, CKD, or dialysis care experience as a main focus of the job.
• Willingness to travel regionally to partner hospitals, clinics, and facilities.

Responsibilities

• Outreach and scheduling face-to-face visits with members in homes, dialysis clinics, and offices.
• Collaborate with nephrologists and care teams to develop individualized care plans.
• Educate patients and families; support transitions of care and readmission prevention.
• Build relationships with partner practices to improve patient outcomes and care coordination.
• Support care management meetings and optimize workflows in aligned care teams.
