Job summary

Somatus is seeking a RN Care Manager to support high-need CKD and ESKD patients through in-home visits, care coordination, and collaborative planning with nephrology practices. You will educate patients, coordinate with physicians, and lead care plan development in a field-based role across assigned markets.

Qualifications

• RN license must be current and transferrable across states as needed.
• 2+ years RN experience in multidisciplinary teams with physicians.
• BL S certification from an approved facility.
• Renal, CKD, or dialysis care experience preferred.
• Residence in a location with reliable high-speed internet or ability to use existing service.
• Willingness to travel to partner hospitals, clinics, and facilities within assigned region (75% travel).

Responsibilities

• Conduct home, provider, or facility visits for care coordination.
• Develop and implement individualized care plans with nephrology teams.
• Coordinate with physicians, PCPs, and care teams to improve patient outcomes.
• Educate patients and families on discharge care and self-management.
• Build strong relationships with provider practices to align workflows and objectives.
• Attend and participate in interdisciplinary care planning meetings.
