Job summary

Somatus is seeking a Registered Nurse Care Manager for a field-based role focusing on high-need CKD and ESKD populations. You will engage with patients in homes, dialysis clinics, and physician offices to plan, educate, and coordinate care with the interdisciplinary team.

You will partner with nephrologists, PCPs, and practice staff to improve outcomes, participate in care planning, and support smooth handoffs across transitions of care in a dynamic environment.

Qualifications

• RN license and ability to get licensed in other states as needed.
• 2+ years RN experience in multidisciplinary settings.
• BLS certification from a recognized facility.
• Renal, CKD or dialysis care focus.
• Willingness to travel within assigned region and reliable internet access.
• Comfort coordinating care with physicians and teams.

Responsibilities

• Outreach and scheduling face-to-face visits at patients’ homes, clinics, or facilities.
• Develop and implement individualized care plans with nephrology and PCP.
• Support care coordination meetings with partner practices.
• Educate patients on discharge requirements and self-management.
• Lead or participate in CMCC meetings and interdisciplinary team activities.
• Provide clinical guidance to non-licensed and licensed team members.
• Travel up to required regional locations for care delivery and planning.
