Job summary

Somatus is seeking an RN Care Manager to support high-need CKD and ESKD patients across the assigned region. You will travel to member homes, clinics, and dialysis centers, coordinating in-person visits and care planning with nephrology practices and primary care teams.

This field-based role requires collaboration with physicians and diverse care team members, strong MS Office skills, and the ability to educate patients on discharge planning and self-management.

Qualifications

• RN license and ability to get licensed in other states as needed.
• 2+ years of RN experience, including working as part of a multi-disciplinary team and Physicians.
• Valid BLS certification from a licensed AHA or American Red Cross training facility or provider.
• Renal, Chronic Kidney Disease or Dialysis Care experience as a main focus of your job.
• Reside in a location that has high-speed internet or can leverage existing service.
• Comfortable traveling to partner hospitals, clinics, and community-based facilities within your assigned region.

Responsibilities

• Outreach and scheduling face-to-face visits with members in homes, dialysis clinics, and physician offices to support high-risk patients.
• Collaborate with nephrology practices to improve patient outcomes and care coordination.
• Provide education and coordination for transition of care and disease management within clinical and home settings.
• Build trusted relationships with providers and act as Somatus’ primary practice representative.
