Qualifications

Responsibilities

• Lead Person Centered Planning process and document member goals.
• Conduct assessments and develop Person Centered Support Plans (PCSP).
• Collaborate with the care team to implement integrated care plans.
• Submit utilization requests and participate in care rounds with medical directors.
