Job summary

Elevance Health is seeking a LTSS Service Coordinator to lead person-centered planning, assessments, and comprehensive care coordination for a designated caseload across long-term services and supports. You will document preferences and goals, develop PCSPs, and ensure regulatory compliance through collaboration with medical directors and an interdisciplinary team.

Qualifications

• Requires BA/BS degree and at least 2 years of social work experience.
• Experience with older adults or individuals with disabilities preferred.
• BA/BS in Health/Nursing preferred.

Responsibilities

• Perform face-to-face program assessments and coordinate waivers.
• Identify members with potential clinical needs and coordinate with care team.
• Document goals in PCSP and develop backup plans.
• Mentor or preceptor for new staff and participate in training.
• Submit utilization/authorization requests with supporting documentation.
• Report critical incidents to state and county agencies.
• Assist with appeals and audits.
