Job summary

Elevance Health is seeking an LTSS Service Coordinator (Case Manager) to manage a designated caseload in specialized programs, conducting assessments and developing Person Centered Support Plans with the member and care team.

You will collaborate with Medical Directors, participate in interdisciplinary rounds, document goals, and advocate for member preferences while ensuring regulatory compliance and high-quality, trauma-informed care in diverse community settings.

Qualifications

• BA/BS degree required; 2+ years in a social work setting.
• Experience with LTSS/IDD or waiver programs is a plus.

Responsibilities

• Manage service coordination for a designated caseload in specialized programs.
• Document member preferences, needs, and goals in PCSPs with care team.
• Collaborate with Medical Directors and participate in interdisciplinary care rounds.
• Mentor or preceptor roles for new staff may be involved.
