Responsibilities

• Perform face-to-face program assessments using predefined tools and motivational interviewing techniques.
• Identify members with potential clinical health care needs and coordinate their cases with the interdisciplinary team.
• Manage non-clinical needs of members to ensure efficient utilization of long-term services and supports.
• Document members' short and long-term goals with their care team and identify changes in service levels or waivers.
• Mentor or preceptor role for new staff and participate in process improvement initiatives.
