Job summary

Elevance Health is seeking a LTSS Service Coordinator to support a defined caseload in the Monroe County area. This field-based role requires travel to client sites, with hybrid work on a policy-based schedule. You will support person-centered planning, coordinate care, and engage with the member’s circle of support.

The position emphasizes coordination of LTSS, BH/PH needs, and collaboration with the care team to deliver integrated care plans under applicable state and contract requirements.

Qualifications

• BA/BS degree required.
• Minimum of 2 years experience with a social work agency or equivalent.
• Specific education/experience may be required by state law and contract.

Responsibilities

• Perform face-to-face program assessments for waiver needs and BH/PH needs.
• Coordinate members' cases with clinical teams as the single point of contact.
• Document goals in PCSP with care team involvement.
• Identify members who may benefit from alternative service levels or other waivers.
• Mentor or precept new staff and participate in process improvements.
• Submit utilization/authorization requests with supporting documentation.
• Report critical incidents to appropriate state/county agencies.
• Assist in appeals, grievances, and state audits.
