Elevance Health is seeking a field-based LTSS Service Coordinator (Case Manager) to lead person-centered planning, assessments, and care coordination across long-term services and supports. You will collaborate with medical directors and an interdisciplinary team to develop integrated care plans and backup strategies in a dynamic community setting.

Competenze

Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency.
Strong preference for case management experience with older adults or individuals with disabilities.
BA/BS in Health/Nursing preferred.

Mansioni

Perform face-to-face program assessments using predefined questions and motivational interviewing techniques.
Identify members with potential clinical health care needs and coordinate cases with the clinical management and interdisciplinary team.
Manage non-clinical needs of members to ensure cost-effective utilization of LTSS/IDD and BH/PH services.
Document goals in collaboration with the member's care team and identify opportunities for alternative levels of service.
May mentor or predefine new staff and participate in process improvement initiatives.
Submit utilization/authorization requests with care plan documentation.
Report critical incidents to state and county agencies as required.
Assist with appeals, grievances, and state audits.
