Job summary

Elevance Health in Columbus, OH is seeking an LTSS Service Coordinator (Case Manager) to manage a designated caseload in specialized programs. You will collaborate with individuals to lead Person Centered Planning, document preferences, conduct assessments, and develop comprehensive PCSPs.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency; or any combination of education and experience which would provide an equivalent background.

Responsibilities

• Perform face-to-face program assessments using predefined questions and motivational interviewing.
• Coordinate and manage waivers (LTSS/IDD), BH or PH needs with clinical teams.
• Manage non-clinical needs of members to optimize utilization of long-term services.
• Document member goals and care plans with care team and caregivers.
• May mentor or train new staff and participate in process improvements.
• Submit utilization/authorization requests with supporting documentation.
• Report critical incidents to state/county agencies as required.
• Assist with appeals, grievances, and state audits.
