Job summary

CareSource seeks an Integrated Care Clinical Manager - Massachusetts. This mobile RN will provide monitoring, follow-up and clinical care management to dual-eligible enrollees with complex medical, behavioral and social needs, coordinating with providers and community resources to improve outcomes and quality of life.

The role requires an MA RN license, case management credentials preferred, and strong collaboration across interdisciplinary teams.

Qualifications

• Associates of Science (A.S) degree in nursing from an accredited nursing program required.
• A Registered Nurse with the ability to independently serve people with complex medical, behavioral, and social needs required.
• Prior experience in care coordination, case management, or working with dual-eligible populations preferred.

Responsibilities

• Engage with enrollees in homes and community settings to build a complex care management relationship.
• Coordinate between healthcare providers, community resources, and enrollees for seamless care transitions.
• Lead interdisciplinary care team to create holistic care plans addressing medical and non-medical needs.
