Commonwealth Care Alliance, Inc. is seeking an Integrated Care Clinical Manager for Massachusetts to supervise and coordinate care for dually-eligible enrollees with complex medical, behavioral, and social needs. This mobile role requires in-home and community visits to ensure seamless care and optimal outcomes.

The position emphasizes interdisciplinary collaboration, care plan development, and follow-up after hospital events to reduce readmissions and improve quality of life.

Qualifications

• AS nursing degree required.
• RN with ability to independently serve people with complex needs.
• Care coordination or case management experience preferred.
• Medicare/Medicaid managed care experience preferred.

Responsibilities

• Engage with enrollees in homes and community settings to build care management relationships.
• Coordinate with healthcare providers and community resources for seamless care transitions.
• Perform comprehensive assessments and develop care plans with updates as needed.
• Lead the interdisciplinary care team to address medical and non-medical needs.
• Oversee enrollee utilization of long-term supports to keep them in their preferred location.
• Educate members about benefits, services, and chronic disease management.
• Follow up after hospitalizations to ensure continuity of care and prevent readmissions.
