Job summary

CareSource seeks an Integrated Care Clinical Manager in Massachusetts to monitor, follow up, and manage care for dually eligible enrollees with complex needs. The role focuses on integrating health services with community resources to improve health outcomes and quality of life.

The position requires leadership of an interdisciplinary team, collaboration with providers and social services, and proactive care planning across settings to ensure seamless, person-centered care.

Qualifications

• Current RN license in Massachusetts.
• Experience in care coordination for dual-eligible populations preferred.
• Case management certification preferred.
• Medicare/Medicaid knowledge a plus.

Responsibilities

• Engage Enrollees in home and community settings to build care plans.
• Coordinate with providers and community resources for seamless care transitions.
• Perform comprehensive assessments and risk evaluations as needed.
• Lead the interdisciplinary care team to create holistic care plans.
• Oversee utilization of long-term services and supports.
• Assist enrollees in accessing housing, transportation, and social services.
• Educate members on benefits and ongoing preventive care.
• Follow up after hospitalizations to prevent readmissions.
• Coordinate with primary care and specialists to share information.
