Job summary

VitalCaring delivers care with comfort and dignity. We are seeking a Field-based Registered Nurse (RN) to lead patient-centered hospice care in homes and facilities, delivering comfort, dignity, and expert symptom management.

You will collaborate with an interdisciplinary team to guide families through end-of-life care, coordinate plans of care, educate caregivers, and ensure compassionate, goal-aligned service with each visit.

Qualifications

• Active RN license (or compact eligibility).
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care.
• Ability to manage a caseload independently.
• Excellent communication in sensitive situations.

Responsibilities

• Lead patient-centered hospice care and manage care plans.
• Coordinate care across the interdisciplinary team.
• Monitor patient condition and adjust plans for comfort.
• Provide education and support to patients and families.
