Job summary

VitalCaring is seeking a Hospice Registered Nurse (RN) to lead and coordinate end-of-life care in field-based settings. You will assess patients, develop comfort-focused plans, and work with physicians, social workers, chaplains, and aides to ensure dignity and quality of life.

You will educate families, manage symptoms, and advocate for patients while maintaining documentation and compliance in a fast-paced home care environment.

Qualifications

• Active RN license or compact eligibility.
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Ability to manage a caseload independently.
• Experience in hospice, palliative care, or home health is a plus.

Responsibilities

• Lead patient-centered hospice care and manage comfort-focused plans.
• Serve as the care coordinator across the interdisciplinary team.
• Provide education, emotional support, and guidance to patients and families.
• Maintain accurate, timely documentation and participate in care planning.
