Job summary

VitalCaring is seeking a Hospice Registered Nurse (RN) to lead compassionate, patient- and family-centered end-of-life care in field-based settings. You’ll build personalized plans, manage symptoms, and advocate for patients’ goals during visits to homes and facilities.

Qualifications

• Active RN license (or compact eligibility).
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care approach.
• Ability to manage a caseload independently.
• Excellent communication skills in sensitive situations.
• Basic EMR proficiency.
• Valid driver’s license and reliable transportation.

Responsibilities

• Lead Patient-Centered Hospice Care: assess patients and families, plan comfort-focused care, and adjust plans as needed.
• Serve as the Care Coordinator: act as case manager and coordinate care with the interdisciplinary team.
• Support Patients & Families: provide education and guidance during the journey; assist in difficult decisions.
• Drive Quality & Compliance: maintain documentation and participate in care planning meetings.
