Job summary

VitalCaring is seeking a Hospice Registered Nurse (RN) to lead patient-centered end-of-life care in homes and facilities.

You will assess, plan, and implement comfort-focused care with an interdisciplinary team You will educate families, coordinate across physicians, social workers, and aides, and ensure compassionate, compliant care while managing a personal caseload and maintaining accurate documentation.

Qualifications

• Active RN license or compact eligibility.
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Basic EMR proficiency and strong clinical judgment.
• Excellent communication in sensitive situations.

Responsibilities

• Lead patient-centered hospice care and plan of care.
• Coordinate care with physicians, social workers, chaplains, and aides.
• Manage symptoms, pain, and comfort goals.
• Educate and support patients and families.
• Document accurately and participate in IDT planning.
