Job summary

VitalCaring is seeking a Hospice Registered Nurse (RN) to lead patient-centered end-of-life care in home and facility settings. You’ll manage symptom control, comfort-focused plans, and support families with empathy and clinical excellence.

This field-based role offers strong autonomy with team collaboration, opportunities to coordinate with physicians, social workers, and aides, and a commitment to dignity, quality of life, and compassionate presence.

Qualifications

• Active RN license (or compact eligibility)
• Graduate of an accredited nursing program
• Minimum 1 year of clinical nursing experience
• Strong clinical judgment and compassionate care approach
• Ability to manage a caseload independently
• Excellent communication skills in sensitive situations
• Basic EMR proficiency
• Valid driver’s license and reliable transportation

Responsibilities

• Lead plan of care and symptom management for hospice patients
• Coordinate care across the interdisciplinary team
• Provide education and emotional support to patients and families
• Document care activities timely and accurately
• Maintain high standards of quality and compliance
