Job summary

VitalCaring is seeking a dedicated Hospice Registered Nurse (RN) to lead and coordinate compassionate, patient-centered end-of-life care in homes and community settings. You will guide care plans, manage symptoms, and support families with empathy and clinical excellence.

As a field-based RN with autonomy and strong team support, you will collaborate with physicians, social workers, chaplains, and aides to deliver comfort and dignity in meaningful moments.

Qualifications

• Must hold an active RN license and graduate from an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care approach.
• Ability to manage a caseload independently and communicate effectively.

Responsibilities

• Lead patient-centered hospice care plans and symptom management.
• Coordinate care with interdisciplinary team.
• Educate and support patients and families throughout the journey.
• Ensure documentation and compliance meet quality standards.
