Job summary

VitalCaring is seeking a Hospice Registered Nurse (RN) for field-based, patient- and family-centered end-of-life care. You will lead care plans, manage symptoms, and advocate for patients and their families with empathy and clinical excellence.

You will collaborate with physicians, social workers, chaplains, and aides, while maintaining accurate documentation and delivering personalized comfort-focused care in homes and community settings.

Qualifications

• Active RN license (or compact eligibility)
• Graduate of an accredited nursing program
• Minimum 1 year of clinical nursing experience
• Basic EMR proficiency
• Valid driver’s license and reliable transportation
• Excellent communication skills in sensitive situations
• Ability to manage a caseload independently

Responsibilities

• Lead patient-centered hospice care: assessments, care plans, pain/symptom management
• Serve as care coordinator: case manager, coordinate care with interdisciplinary team
• Support patients and families: education, emotional support, and guidance
• Drive quality and compliance: documentation, IDT participation, standards adherence
