Job summary

VitalCaring is seeking a Hospice Registered Nurse (RN) to lead patient-centered care in field-based settings, delivering comfort and dignity at end of life. You will guide care plans, manage symptoms, and advocate for patients and families with empathy and clinical excellence.

The role emphasizes autonomy with support from an interdisciplinary team and a strong focus on quality, documentation, and compassionate presence in homes and facilities.

Qualifications

• Active RN license (or compact eligibility) required.
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.

Responsibilities

• Lead patient-centered hospice care by assessing patients and families.
• Develop and manage individualized care plans focused on comfort.
• Coordinate care with an interdisciplinary team including physicians, social workers, chaplains, and aides.
• Educate and support families throughout the journey and help with decisions.
• Maintain accurate documentation and participate in care planning and compliance.
