Job summary

OneWell Health Care in Quakertown, PA is seeking a dedicated Job Coach to support individuals with IDD in securing and maintaining competitive employment in the community. This field-based role provides on-site coaching at employer worksites to foster independence and long-term job success.

As part of the IDD team, you will deliver hands-on training, coordinate with employers, document progress, and ensure compliance with DHS, ODP, and HCBS requirements while promoting inclusive, person-centered

Qualifications

• High school diploma or GED required.
• Bachelor's degree in Human Services, Education, Psychology or related field preferred.

Responsibilities

• Provide on-site job coaching at employer worksites to support skill acquisition and independence.
• Support individuals in achieving employment goals and maintaining competitive employment.
• Coordinate with employers and employment teams; assist with transportation and travel training when applicable.
• Document progress and ensure compliance with ODP, DHS, HCBS and billing standards.
• Promote inclusive, person-centered employment outcomes and maintain confidentiality.
