Job summary

Humana is seeking a Case Manager to serve Wisconsin Family Care members, focusing on frail elders and adults with disabilities. You will coordinate care with a Field Care Nurse, travel to member homes for assessments, and maintain detailed MCP documentation.

Qualifications

Bachelor's degree in human services or related field with 1 year of experience serving frail elders or adults with disabilities, or a Bachelor's in another field with 3+ years of such experience.
Valid state driver's license and proof of vehicle liability insurance with 100/300/100 limits.
Ability to travel up to 40% of time to visit members.

Responsibilities

Assess members' health and safety needs to develop a comprehensive Member Care Plan (MCP).
Coordinate with a Field Care Nurse to address health and safety needs within MCP.
Travel to members' location for assessments and visits (up to 40%).
Arrange support services including social integration, housing, and other non-medical needs.
Maintain accurate documentation including case notes and MCP updates.
