Job summary

Humana, Inc. is seeking a Case Manager to serve members in the Wisconsin Family Care program from Eau Claire, WI. You will assess needs, coordinate with a Field Care Nurse, and develop a comprehensive Member Care Plan for frail elders and adults with disabilities.

The role requires travel up to 40% and a valid driver’s license. Work location is Eau Claire, WI with typical hours Monday–Friday, 8:00 am–4:30 pm CST. Humana emphasizes a patient-centered care approach.

Qualifications

• Bachelor's degree in human services or a related field required, with 1 year of relevant experience.
• Alternatively, a Bachelor's in another field with 3 years of relevant experience.
• Valid state driver's license and proof of auto liability insurance (100/300/100).

Responsibilities

• Assess health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to provide services in the least restrictive environment per MCP.
• Travel to members' locations for face-to-face assessments at enrollment and at least every six months.
• Travel to members' homes for quarterly in-person visits and monthly phone contact.
• Arrange support services including social integration, community resources, housing, and other non-medical needs.
• Ensure cost-effective service delivery and maintain documentation incl. MCP and authorizations.
