Job summary

Humana is seeking a Case Manager to support Wisconsin Family Care members, focusing on frail elders and adults with disabilities. You will assess needs, develop a care plan, and coordinate with a Field Care Nurse to provide services in the least restrictive environment.

You will travel to members' homes for enrollment assessments and quarterly visits, with monthly phone contact. Workdays are typically Mon–Fri, 8:00 am–4:30 pm CST; mileage is reimbursed.

Qualifications

• Bachelor's degree in human services or related field with 1 year of relevant experience; or 3 years of relevant experience with a different degree.
• Travel up to 40% and valid driver's license with liability insurance.
• Knowledge of community health and social service agencies.

Responsibilities

• Assess health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to deliver services in the MCP.
• Travel to members' locations for enrollment assessments and periodic visits.
• Conduct quarterly in-person visits and maintain monthly contact by phone.
• Arrange non-medical supports including housing, resources, and social integration.
• Evaluate risk factors and provide member education.
