Job summary

Humana is seeking a Case Manager to support Wisconsin Family Care members, assessing health and safety needs and developing a comprehensive Member Care Plan. You will coordinate with a Field Care Nurse to deliver services in the least restrictive environment and travel to member homes for assessments and visits.

Qualifications

• Bachelor's degree in human services or related field required.
• At least 1 year of experience serving frail elders or adults with disabilities.
• Alternative: Bachelor's in another field with 3+ years of such experience.
• Valid state driver's license and proof of personal vehicle liability insurance with at least 100/300/100 limits.

Responsibilities

• Assess members' health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with Field Care Nurse to address health and safety needs within MCP and least restrictive environment.
• Travel to member locations for face-to-face social assessments at enrollment and at least every six months.
• Travel to member homes for quarterly in-person visits and maintain monthly phone contact.
• Arrange support services including social integration, community resources, employment, housing, and non-medical needs.
• Evaluate risk factors and provide education to members.
