Job summary

VitalCaring Group is seeking a Care Transition Coordinator to connect patients, families, and healthcare partners with hospice and home health services. This field-based role emphasizes relationship-building, sales calls, and seamless care transitions.

You will collaborate with hospitals, physicians, and care teams to grow referrals and ensure timely admissions while maintaining accurate CRM/EMR documentation. A healthcare background and a valid driver's license are required.

Qualifications

• Bachelor’s degree in Healthcare, Business, Marketing, or related field preferred (or equivalent experience).
• Minimum of two years of healthcare or related sales experience.
• Strong communication, relationship-building, and influencing skills.
• Ability to manage multiple priorities in a fast-paced, field-based environment.
• Proficiency with Microsoft Office and CRM systems.
• Demonstrated ability to work independently and drive results.
• Valid driver’s license and reliable transportation.

Responsibilities

• Serve as the primary liaison between referral sources, patients, caregivers, and clinical teams to ensure seamless care transitions.
• Execute structured, effective sales calls including pre-call planning, relationship building, needs discovery, and closing.
• Build and maintain strong relationships with referral partners through consistent communication and service excellence.
• Develop and execute strategies to grow market share and drive referral volume.
• Track, manage, and document sales activity across referral sources using CRM tools.
• Achieve established referral and admission goals.
