Job summary

Elevance Health is seeking an LTSS Service Coordinator (Bilingual in Spanish/English) to coordinate long‑term supports for a defined caseload. The role is primarily field-based across multiple Florida counties, with occasional office visits and hybrid work expectations.

You will conduct assessments, support Person Centered Planning, and collaborate with the care team to develop integrated care plans addressing PH/BH/LTSS needs and member goals.

Qualifications

• BA/BS degree required or equivalent
• Minimum 2 years of experience with a social work agency
• Bilingual in English/Spanish strongly preferred

Responsibilities

• Conduct face-to-face program assessments using defined tools and questions.
• Coordinate and manage waivers such as LTSS/IDD and BH/PH needs within a care team.
• Document member goals and care plans in collaboration with care partners.
• Identify members who may benefit from alternative levels of service or other waiver programs.
• Provide support in audits, appeals, and reporting to ensure compliant care coordination.
