Job summary

Elevance Health is seeking a LTSS Service Coordinator (Bilingual in Spanish/English) for a field-based role, primarily serving Florida counties. The position requires residency within specified counties and regular travel to client sites with occasional office attendance.

The coordinator will conduct assessments, facilitate Person Centered Planning, and support a fully integrated care plan (PH/BH/LTSS). Collaboration with care teams, family, and physicians is essential to success.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency.
• Specific education, years, and type of experience may be required by state law and contract.
• Bilingual in English/Spanish strongly preferred.

Responsibilities

• Perform face-to-face program assessments using predefined tools and conduct motivational interviewing for evaluations.
• Identify members with potential clinical health care needs and coordinate cases with the clinical health management and interdisciplinary team.
• Manage non-clinical needs of members with chronic illnesses to ensure efficient LTSS utilization.
• Document short and long-term service goals with the member and care team.
• Identify members who would benefit from alternative levels of service or other waiver programs.
• May serve as mentor or preceptor for new staff and participate in process improvements.
• Submit utilization/authorization requests aligned with the care plan.
• Report critical incidents to appropriate state and county agencies.
• Assist with appeals, grievances, and state audits.
